Home and Community Carehome care

The Home and Community Care Program (HACC) provides a broad range of flexible support services to assist frail older people, people with functional disabilities and the carers of these people to -

  • Continue to live independently in the community, reducing the risk of premature admission into residential care;
  • Build independence and empower them to undertake activities of daily living.

Eligibility

Priority is given to people who have high support needs, frail aged people, people with a permanent disability, and carers of these people. Not all residents referred for services may be eligible for HACC services. Eligibility will be determined in accordance with Home & Community Care guidelines.

Referrals

Referrals for Home and Community Care services may be accepted from all sources including self referral, family, doctor, neighbours and friends. If you are making a referral for someone else you must first gain their permission to provide their personal details to the intake worker.

Assessment

New clients will require an assessment before services commence.
Our Home and Community Care Assessment Officers will conduct an in home assessment to determine the clients’ individual needs. A flexible, tailored care plan will be prepared before services are implemented.

Cost

Fees are charged for HACC services in accordance with HACC guidelines and will be discussed at assessment.
